MD on the Net is available for your use under the terms and conditions established by the board of directors of MD on the Net, Inc. governed by the laws of the state of Indiana. By clicking the I AGREE button, you acknowledge that you have read, understood and will abide by the provisions set forth by the agreement. MD on The Net reserve the right to make modifications to these terms and conditions at ant time without prior notification, and whatever modifications made will become effective once publicly posted. Members reserve the right to disenroll without penalty if they disagree with modifications made otherwise, whatever modifications made shall be deemed governance under the original conditions agreed to. MD on The Net also reserves the right to make as many modifications as it deemed necessary for the successful operation of the company.

MEDICAL USE

MD on the Net is a medical dialogue site on the internet and does not and will not assume to be the substitute for well established patient-physician relations. Any relationship established between MD on The Net and its members shall be condoned, remain confidential, personal and professional. All medical advice given shall come from a board-certified physician qualified to make such recommendation.

PRIVACY

Every effort will be made to provide and maintain at all times privacy to all our members. Information and data obtained are for the private use of MD on the Net Physicians and authorized personnel only and shall under no circumstances be sold to any entity or entities. In case of any security breach, all efforts would be made to identify the perpetrators and legal action will be taken to the full extent of the law. Integrity is the key at MD on The Net.

SUBCRIPTIONS

All subscriptions to MD on The Net are purely voluntary without coercion and the selection of payment plan made is with full understanding of its intent. Members are responsible for the creation and safe maintenance of username and password and to safeguard them within their possession. A correct username and password entry of an active member will be judged by MD on The Net as the owner, and all access will be given to that user. We encourage all our members to routinely change password to maintain better confidentiality.

PAYMENT

All payments to MD on The Net are via an on-line credit card mechanism. Payments are the sole responsibility of the member. As MD on The Net has not yet contracted with any insurance carrier, any service or services currently rendered shall be the sole responsibility of the member. No excess or unnecessary charges shall be made by MD on The Net on members' credit cards and any charge made shall first be selected by the member.

APPLICABLE LAW

All disputes either arising from these terms and condition or between MD on The Net and a member or entity shall be governed by the laws of the United States of America and specifically, that of Indiana. Lake County is the selected jurisdiction to settle all disputes. To the full extent permitted by law, every attempted shall be made to settle all disputes through arbitration and the member waives all rights to a jury trial in all disputes arising from these term and condition.

NOTICE AND TERMINATION

All notices to the members shall be made through the e-mail provided by the member and shall be effective on the date posted. All notices to MD on The Net shall be made in writing and are effective on the date received and stamped. Comments can be made by member to MD on The Net officials by clicking on "CONTACT US". All mails to MD on The Net shall be on first class postage prepaid in US mail or via special couriers arranged by the member. Either MD on The Net or the member may terminate these terms and condition without cause and therefore effectively terminate member's access to the member only sections of the website. Failure to make appropriate payments or abide by these terms and conditions shall constitute grounds for termination by MD on The Net.

LANGUAGE

Use of obscene, vulgar, racially insensitive, discriminatory languages are neither condoned nor permitted on this website.

ASSIGNMENT AND SEVERABILITY

Members may not assign this agreement. MD on The Net may assign all or parts of its right under these term and condition to its affiliate, subsidiaries or Successor Corporation. The provisions of these terms of use are severable and in the event any part of the terms and conditions are deemed invalid or illegal and unenforceable, the remaining provisions shall become invalid and unenforceable. This agreement supersedes all prior agreement of any kind whether written or implied and shall remain in effect until terminated or changed by MD on The Net.
